Adding Microsoft Surface Mouse to Tooth Fairy

I own and love a Microsoft Surface Mouse. It’s the bluetooth mouse of choice for me on my Mac Mini, but I cannot add it to Tooth Fairy. Any ideas how to get this added? The ultimate goal is to get a hotkey to connect it when I boot up my Mac in the morning. It would be GREAT to get this working properly, but if you don’t have any suggestions I will begrudgingly switch to another bluetooth mouse.

The Select button being disabled is caused by a macOS bug (FB20376419, FB20710035). Did you try the workaround suggested in the dialog, to Option-click the + button in ToothFairy and enter the device’s Bluetooth address?
